It's not just about saying USA only in your listing, unless your policies are set to ONLY ALLOW us buyers even if you say US only but have your settings for global your still screwed so check your settings if you don't want to ship internationally.
As for how to handle it, its really up to you. If your ebay feedback is really important to you then take the hit and ship the item and take the loss yourself and take it as a lesson learned. If you dont care as much and are willing to roll the dice tell the buyer what happenned, tel them what hte actual shipping costs are and see if they are willing to pay or maybe split the difference. Can't hurt to ask even if they say no you can still deny the sale if thats what you want to do.
Are you just selling some stuff on the side or are you trying to be a businessman? If your trying to be a business man you made the mistake, you honor the sale and take the loss. If your just hustling on the side and don't want to be a true business then you can back out of the sale
I recently saw someone selling something and I realized the price was listed wrong. I bought on purpose just to see what would happen as it was an item I was interested in. I bought three items, the guy honored the sale and took a $375 loss, I know that for a fact because i know what the cost of his items is because the nature of the items its a fixed price ie silver and currency.
